  • Personal Medication List: A concise chart to help clients understand what medicines they are taking, why they are taking them, how they should be taken, how they work, how they are monitored, and potential side effects
  • Client Action Plan: A list of steps clients can take to increase medication efficacy and improve their health
  • Summary of Recommendations: Suggested drug regimen modifications for clients to discuss with their provider(s)
